  • Preventive Eligibility & Family History (BMI < 25): While preventive MNT is automatically covered for individuals classified as overweight or obese (BMI of 25>) or diagnosed with pre-diabetes/cardiovascular risk, clients with a BMI under 25 can still qualify for $0-cost preventive nutrition counseling if they have a personal or family history of cardiovascular disease, early heart disease, hypertension, high cholesterol, or Type 2 diabetes.

Step-by-Step Guide: How to Verify Your UHC Benefits

We recommend calling UnitedHealthcare Member Services before your first visit to confirm your plan details:

  1. Call Member Services: Dial the telephone number listed on the back of your UHC member ID card. Ask to speak with a representative regarding Outpatient Nutrition Counseling / Medical Nutrition Therapy (MNT).

  2. Provide CPT Procedure Codes: Ask if your plan covers CPT codes 97802 (Initial Assessment, 15-min increments) and 97803 (Re-evaluation/Follow-up, 15-min increments).

  3. Confirm Preventive vs. Diagnostic Coverage: Ask: "Is nutrition counseling covered as a preventive benefit under diagnostic code Z71.3 (Dietary Counseling)? Is there a $0 copay or deductible requirement?"

  4. Confirm Visit Limits: Ask how many nutrition sessions your plan allows per calendar year (e.g., unlimited, 3, 6, 12, or combined limits).

  5. Confirm Telehealth Eligibility: Ask: "Are virtual/telehealth visits (CPT codes with modifier 95 or place of service 02/10) covered at the same rate as in-person visits?"

  • Yes, most commercial UnitedHealthcare PPO, POS, and employer-sponsored plans offer coverage for nutrition counseling and MNT when rendered by a licensed, in-network Registered Dietitian.

  • Visit allowances depend on your individual group plan. Many preventive policies cover unlimited sessions, while other group plans specify an annual allotment (such as 3, 6, 12, or 26 visits per year). Calling member services will reveal your exact visit limit.

  • UnitedHealthcare is a national network. As long as your plan includes out-of-state or national PPO/Choice Plus network access and your dietitian is licensed in the state where you reside during your session, billing is processed directly through UHC's nationwide network.

  • We accept standard commercial UnitedHealthcare plans, including UHC Choice, Choice Plus, Options PPO, Navigate (with referral), GEHA (under the UHC network), and third-party UHC administrators like UMR.

  • Most UnitedHealthcare PPO and Choice Plus plans do not require a physician's referral to see an in-network Registered Dietitian. However, select HMO, Navigate, or specialized plans may require a referral or a preventive MNT order from your primary care provider.

  • We require at least 24 to 48 hours' advance notice to cancel or reschedule appointments without fee. Because insurance only reimburses for completed sessions, late cancellations or missed visits may incur a standard non-refundable late fee.
