Pediatric Care (Under Age 21 / EPSDT): Children and young adults under age 21 are broadly covered for nutrition counseling (growth issues, allergies, pediatric obesity, or general wellness) under Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) guidelines without adult BMI restrictions.
Adults (Age 21 and Older):
BMI 30 or Greater / Covered Conditions: Direct scheduling is available for adults with a BMI of 30 or those diagnosed with Type 2 Diabetes, Pre-Diabetes, Hypertension, Cardiovascular Risk, or Kidney Disease.
BMI Under 30: Adults with a BMI under 30 require a signed doctor’s referral and a Prior Authorization (PA) submitted by our team to your plan before booking.
Doctor's Referral: If required based on your age and BMI, ask your doctor, Physician Assistant (PA), or Advanced Registered Nurse Practitioner (ARNP) to complete a referral for Medical Nutrition Therapy (CPT 97802/97803).
Accepted Plans Only: We accept major Washington Apple Health Managed Care Organizations (MCOs). (Note: We do not accept Wellpoint).

1. Will I have to pay anything out-of-pocket?
No. Eligible Washington Apple Health beneficiaries pay $0 out-of-pocket ($0 copay, $0 deductible, $0 balance billing) for covered nutrition visits.
2. Do I need a doctor's referral to schedule?
A doctor's referral and Prior Authorization are specifically required for adults (age 21 and older) with a BMI under 30. For adults with a BMI of 30+ or children under 21 (EPSDT), a referral or prior authorization is not required.
3. What if I am 21 or older and my BMI is less than 30?
If you are 21 or older with a BMI under 30, your doctor's referral and medical notes must be submitted to your Apple Health plan for Prior Authorization (PA) approval before your appointment can be scheduled.
4. Do you offer Spanish-speaking dietitians?
Yes. We have native Spanish-speaking Registered Dietitians and clinical specialists available.
5. What AppleHealth medicaid plans do you accept?
We accept Molina, CHPW, Coordinated Care, UnitedHealthcare Community Plan, and ProviderOne, but we do not accept Wellpoint Apple Health.
6. What if I need to cancel or reschedule?
Please notify us as soon as possible if you need to change your appointment. In accordance with Washington Medicaid law, you will never be charged a no-show fee or late cancellation fee.
